HPV-DeepSeek is an innovative test capable of identifying HPV-associated head and neck cancer years before the first symptoms appear. This method is based on detecting fragments of viral DNA in the blood, which could significantly change current approaches to early diagnosis and treatment of such tumors.

HPV and Its Role in Cancer Development

Human papillomavirus (HPV) is responsible for about 70% of head and neck cancer cases in the United States, making it one of the leading causes of this disease. Unlike cervical cancer, which can be detected through regular screening, there is currently no test that allows for the detection of HPV-associated head and neck cancer before symptoms arise. As a result, most patients only learn of their diagnosis when the tumor has already grown significantly and spread to the lymph nodes, causing noticeable symptoms. Finding ways to detect these cancers earlier could enable timely treatment and improve patient outcomes.

Breakthrough Research and Test Development

A new study, funded by the U.S. federal government and published in the Journal of the National Cancer Institute, was conducted by scientists from Mass General Brigham. The team developed the HPV-DeepSeek liquid biopsy test, which can detect HPV-related head and neck cancer up to 10 years before symptoms appear. Such early detection could increase the chances of successful treatment and reduce the need for aggressive therapies. The researchers note that their work is the first to demonstrate the possibility of accurately identifying HPV-associated cancer in asymptomatic individuals many years before diagnosis. Typically, by the time symptoms develop, patients require treatments that often come with serious and lifelong side effects. Tools like HPV-DeepSeek are expected to enable detection at the earliest stages, ultimately improving patient outcomes and quality of life.

How HPV-DeepSeek Works

The test uses whole-genome sequencing to detect trace fragments of HPV DNA that have separated from the tumor and entered the bloodstream. Previously, the team showed that the test could achieve 99% specificity and 99% sensitivity in detecting cancer at the initial clinical stage, outperforming all existing diagnostic methods.

Study Results

To determine whether HPV-DeepSeek could detect cancer long before symptoms appear, researchers analyzed 56 blood samples from the Mass General Brigham biobank: 28 samples from people who later developed HPV-associated head and neck cancer, and 28 from healthy individuals (the control group).

HPV-DeepSeek detected HPV tumor DNA in 22 out of 28 blood samples from patients who later developed cancer, while all 28 control samples tested negative, indicating high test specificity. The test was more effective at detecting HPV DNA in samples taken closer to the time of diagnosis, with the earliest positive result found in a sample collected 7.8 years before diagnosis. Using machine learning methods, the researchers further improved the test’s accuracy: it correctly identified 27 out of 28 cancer cases, including samples collected up to 10 years before diagnosis.

Currently, the authors are confirming these results in a second blinded study funded by the National Institutes of Health (NIH), using hundreds of samples collected as part of the National Cancer Institute’s Prostate, Lung, Colorectal, and Ovarian (PLCO) Cancer Screening Trial.
