Overall, mouthwashes with a high concentration of garlic extract demonstrated antimicrobial results similar to those of chlorhexidine. Effectiveness depended on the concentration of the product and the duration of use, which influenced differences in outcomes. Some studies favored chlorhexidine for maintaining a higher pH level, while others noted greater effectiveness of garlic at certain concentrations. However, garlic-based mouthwash may cause more discomfort.

Side Effects and Usage Considerations

Although garlic extract can cause side effects, they are generally less pronounced than those of synthetic chemical agents. Reports include burning sensations and an unpleasant odor, which may affect patients' willingness to switch from chlorhexidine to plant-based alternatives.

Despite some limitations, the review confirms the significant clinical antimicrobial activity of garlic extract, noting a marked reduction in bacterial counts compared to baseline. This suggests that garlic-based mouthwash could be considered a possible alternative to chlorhexidine in certain cases. The authors recommend further clinical studies with larger participant groups and longer observation periods to confirm effectiveness and improve the clinical applicability of garlic as an alternative to chlorhexidine, which remains the standard among antimicrobial mouthwashes.

The review highlights numerous studies demonstrating the antimicrobial effects of garlic extracts against a broad spectrum of bacteria, fungi, and viruses. Garlic extract has also been studied for treating denture stomatitis, disinfecting dentinal tubules, and as an intracanal medicament. These studies have contributed to understanding garlic’s antimicrobial role compared to synthetic agents like chlorhexidine. However, most research has been conducted in vitro, used different methodologies, and lacked clinical standardization, underscoring the need for further studies to confirm garlic’s effectiveness in dental practice. This gap remains significant in the evidence-based antimicrobial therapy literature.
