Comparing Risk Assessment Methods

A recent study showed that calcium scanning is most effective for certain groups of patients. Observing more than 6,000 adults over a 10-year period revealed that adding the calcium score to the standard PREVENT cardiovascular risk calculator from the American Heart Association only slightly improved risk prediction for the overall study population.

PREVENT estimates the probability of developing cardiovascular diseases over 10 or 30 years based on commonly available health data such as blood pressure, cholesterol levels, age, and sex. Coronary calcium scanning uses a different approach: CT scans detect calcified plaques in the coronary arteries, and the resulting score reflects their quantity. A higher score is associated with an increased risk of future cardiovascular events.

Greatest Benefit for the Intermediate Risk Group

The study found that the calcium score is especially helpful for patients whom PREVENT classifies as being in the borderline or intermediate risk category. For these individuals, additional scanning allows for a more accurate assessment of their likelihood of developing cardiovascular diseases.

Limitations and Potential Drawbacks

Routine use of calcium scanning in people with low risk may lead to unnecessary radiation exposure, extra tests, and costs without clear clinical benefit. For patients at high risk, scanning is also not always justified, since they are recommended to start statin therapy regardless of test results.

Study Design

The study included more than 6,000 adults aged 45 to 79 who participated in a multi-ethnic atherosclerosis study. At the start, each participant had their calcium score determined and their 10-year risk calculated using PREVENT. Over the following decade, 6% of participants experienced a heart attack or stroke. Comparing predictions that included the calcium score with those based only on PREVENT showed only a modest overall improvement in accuracy: the model’s ability to distinguish people who would have future cardiovascular events increased from 0.73 to 0.75.

However, for the group with borderline or intermediate risk (3–9% risk over 10 years), adding the calcium score provided a more significant improvement in risk prediction.

The Importance of Accurate Risk Assessment

About 10% of adults in the U.S. aged 30–79 have cardiovascular diseases, which remain the leading cause of death in the country. Many heart attacks, strokes, and other cardiovascular events can be prevented. Accurate risk assessment helps doctors determine who will benefit most from preventive treatments, including statins that lower cholesterol and reduce cardiovascular risk.

The Need for Further Research

Additional studies are needed to clarify the benefits of the calcium score in different populations, including higher-risk groups such as adults of South Asian and Filipino descent. More data are also needed on younger people, since participants in this study were aged 45 to 79 at the time it began.
